Propel Orthodontics, LLC or its successors or assigns (“Propel Orthodontics” or “we”) operates www.propelorthodontics.com (“Propel Orthodontics” or “Site”). This Privacy Policy describes Propel Orthodontics’ use and sharing of personally identifiable information that users voluntarily provide to Propel Orthodontics when they register for a trial or full-time basis. The term “User” refers to a visitor or a registered User. Please read this privacy policy (the “Policy”) to learn more about how we access, use, store and treat your personally-identifiable information in conjunction with the Propel Orthodontics services (“Services”).
If you have any hesitation about providing Propel Orthodontics any information and/or having such information displayed on Propel Orthodontics or otherwise used in any manner permitted in this Policy and your User Agreement, you should not input any information into Propel Orthodontics and should not use the Services.
Propel Orthodontics adheres to the following principles to protect your privacy when using Propel Orthodontics:
- We do not use personally identifiable information (“PII”) for marketing purposes, except for voluntary testimonials, as described below.
- If you choose to stop using the Propel Orthodontics Services, select Account Setting and Unsubscribe. Once you unsubscribe, we permanently remove all your personal information from Propel Orthodontics.
The Services have special provisions for use by children and minors. Propel Orthodontics Services is a general audience service and does not knowingly collect PII from children under 13 years of age. Anyone under the age of 13 should refrain from accessing or using our Services. Anyone between ages 13 and 18 must have this policy reviewed and accepted by a parent or legal guardian.
We reserve the right to modify this Policy at any time, so please review it frequently. Further details about our Services and our commitment to privacy are identified below.
Periodically, Propel Orthodontics may change this Privacy Policy to reflect changes in the law, our PII collection and use practices and the features or Services of Propel Orthodontics which shall be effective upon posting by Propel Orthodontics on Propel Orthodontics. Your continued use of the Services after Propel Orthodontics posts a revised Privacy Policy reflects your acceptance of the revised Privacy Policy. Thus it is important that you review this Privacy Policy frequently.
1. Information Collection
1.1 In order to use the Services, you must join Propel Orthodontics and create an account with us. To create the account, we ask that you provide PII, which may include, among other items, your name, email address, gender, birthday, location, company. Certain fields of information are required information, but other information will be optional. You agree to only put the applicable information into each field and to ensure its accuracy.
1.2 Like other web services, we use cookies and weblog files to improve your experience of the Services. We do not store PII in the cookies. You can program your computer to warn you each time a cookie is being sent, block third party cookies or block all cookies. If your computer settings do not allow cookies, you will not be able to utilize the Services.
1.3 Third-party advertisements displayed on Propel Orthodontics may also contain cookies set by Internet advertising companies or advertisers (known as “third party cookies”). Propel Orthodontics does not control these third-party cookies and Users of the Services should check the privacy policy of the Internet advertising company or advertiser to see whether and how it uses cookies.
1.4 A pixel tag is a tiny image inserted in a webpage and used to record the number and types of views for that page. Propel Orthodontics may allow third party pixel tags to be present on Propel Orthodontics for purposes of advertising, providing services or data and statistics collection.
1.5 Some of the advertisements that appear on Propel Orthodontics may also be delivered to you by third-party Internet advertising companies. These advertisers use certain technologies to deliver advertisements and to collect non-PII about your visit to or use of Services, including information about the ads they display, via a cookie placed on your computer that reads your IP address.
1.6 Third-party applications (such as widgets) created by third-party developers may be available on the Services. Third-party applications are small bits of software, often with interactivity, that can be installed into Registered Users’ profiles or shared with other Users. However, Propel Orthodontics does not control the third-party developers, and cannot dictate their actions. When a Registered User engages with a third-party application, that Registered User is interacting with the third-party developer, not with Propel Orthodontics. We encourage Registered Users not to provide PII to the third party’s application unless the Registered User knows the party.
1.7 Due to Internet standards, when you access the Services, we automatically receive information regarding the website you came from prior to Propel Orthodontics and the website you go to when leaving Propel Orthodontics. We also receive the IP address of your computer (or the proxy server you use to access the Web), your internet services provider, and your computer’s operating system. Any linkage between your IP address and your PII is not shared with independent third parties, except with your permission or as required by law.
1.8 Certain information we obtain may reveal or allow others to identify aspects of your private life, including information deemed “sensitive” by applicable law. Please be aware that in providing information to us, you expressly and voluntarily accept this Policy and the terms of your User Agreement. You may terminate the Services and your consent to this Policy and the terms of your User Agreement, however, any such withdrawal will not be retroactive.
1.9 Services may be linked to Internet sites operated by other companies. Propel Orthodontics may also carry advertisements from other companies. Propel Orthodontics is not responsible for the privacy practices of websites or other services operated by third parties that are linked to or integrated with the Propel Orthodontics Services or for the privacy practices of third party Internet advertising companies. Once you leave Propel Orthodontics via such a link, access a third-party application (such as widgets) or click on an advertisement, you should check the applicable privacy policy of the third party or advertiser site to determine, among other things, how they will handle any PII they collect from you.
2. Use of Information
2.1 To provide worthwhile results and a better experience to you, we use your information to improve upon the Services.
2.2 We do not sell or rent your PII to third parties for marketing purposes. We share PII with third parties solely to carry out your instructions or to provide specific services or obtain specific information.
2.3 We may ask you to comment about the Services (“Testimonials”). Any comments are entirely voluntary, but if you do provide Testimonials, you acknowledge and agree that in connection with the promotion of our offerings to third parties, we may use your Testimonials along with your name and other personal information that you have provided.
2.4 We may need to disclose your PII when required by law or if we have a good faith belief that such action is necessary to (a) comply with a current judicial proceeding, a court order or legal process served on Propel Orthodontics, (b) enforce this Policy or the User Agreement, (c) respond to claims that your PII violates the rights of third parties; or (d) protect the rights, property or personal safety of Propel Orthodontics, its users and the public. You authorize us to disclose any information about you to law enforcement or other government officials as we, in our sole discretion, believe necessary or appropriate, in connection with an investigation of fraud, intellectual property infringements, or other activity that is illegal or may expose Propel Orthodontics or you to legal liability.
3. Use of Information.
The information gathered from your use of our Site or provided by you may be used:
3.1 To provide you with information about products, services, news and events.
3.2 To allow you to use, purchase, and/or download products and services.
3.3 To gather demographic information about user trends, such as age, gender, and general income levels.
3.4 To provide certified doctors with information on actual or prospective patients.
3.5 To analyze the use of our services and products, develop new services and products, and customize our products, services, and other information we make available.
We may also disclose personal information about you if we believe that doing so is legally required or is necessary to protect our property or other legal rights (including but not limited to enforcement of our agreements or the rights or property of others).
We partner with other businesses to assist us in our marketing, communications, and sales efforts and may share information about you for these purposes. These partners are not allowed to use your information for any purpose other than doing business with Propel Orthodontics.
In addition, information about our users, including personal information, may be disclosed as part of any merger, acquisition, debt financing, sale of company assets, or similar transaction, as well as in the event of insolvency, bankruptcy or receivership in which personal information could be transferred to third parties as one of our business assets.
4. Choices Regarding Your Information.
4.1 You can review the personal information you provided Propel Orthodontics and update such information. If you update any of your information, we may keep a copy of the information which you originally provided to us in our archives for uses documented herein. 4.2 You can close your account through the Services by clicking on the Unsubscribe button in the Account Settings section and your account and all information therein will be deleted. However, as part of our archival processes, we may retain certain data contributed by you if we believe it may be necessary to prevent fraud or future abuse, or if required by law.
